Virtual Clinic

CCCHC offers virtual care services Monday - Friday, 8:00 a.m. - 5:00 p.m. You will need MyChart access, strong internet connection and a computer, tablet or cell phone device with camera and audio. Schedule an appointment today through MyChart or by calling the clinic.

TytoCare

CCCHC utilizes a medical device called TytoCare to assist in performing medical exams with a provider through virtual visits. The TytoCare device provides your CCCHC provider with high-quality digital sounds of the heart and lungs and high-quality digital images and video of the ears, throat, and skin. The device also measures heart rate and body temperature.

Remote Patient Monitoring

Remote patient monitoring is a form of virtual care that allows a CCCHC provider to monitor and manage their patients’ chronic conditions. It can also prevent health complications in patients who aren’t able to easily travel.

There are many symptoms and conditions that can be tracked through remote patient monitoring, including:

  • High blood pressure
  • Diabetes
  • Weight loss or gain
  • Heart conditions
  • Chronic obstructive pulmonary disease
  • Sleep apnea
  • Asthma
