Are you on track? Coal Country Community Health Center is offering backpacks filled with fun! Children ages 3‐6 years old must complete their Well Child Check in the month of September to receive a backpack. Call to schedule an appointment for your child at any of the CCCHC Clinic locations!

Registration forms are located at Coal Country Community Health Center. $20/per adult, $15/ages 6-12 and ages 5 and under: FREE. Registration fee includes Oliver County Fair Pass for Saturday.

Call to schedule your sports physical appointment at CCCHC today! Bring the Sports Physical Form with and have the Parent/Athlete section completed. This form is provided by your school or visit www.ndhsaa.com to print a form. Parents please accompany your student athlete.

Talk to your doctor or other healthcare professional to make sure your children get the vaccinations they need when they need them. If you haven’t already, check your child’s immunization record and schedule a visit to their physician or clinic. Doing so now will avoid a potential last‐minute rush and will help make sure there are no surprises on the first day back to school.
