Sports Physicals + Wellness Exams at School Clinics

Coal Country Community Health Center will be holding preventative wellness exams for students in PreK-12th grade this Spring at our Beulah and Hazen School Clinics. A wellness exam may include a sports physical for the 2026-2027 school year if requested. The sports physical will be valid through the 2027 school year. Parents/guardians should check insurance eligibility prior to scheduling.

Impact testing is available for incoming 11th graders on May 6 & 13 from 12:00 - 2:30 p.m with Amber Blackhurst, RN at the Beulah School Clinic.

Important Reminders

You must bring the youth wellness/sports physical student packet to appointment with all sections completed.
This is a great opportunity to update immunizations! A parent or guardian can call before the exam or accompany their child for consent of updating immunizations.
