Coal Country Community Health Center will be holding Sports Physical Clinics at all clinic locations in July. Athletes participating in any sport during the 2021-2022 school year are encouraged to schedule an appointment.
- CCCHC will collect a $25.00 fee at the time of the appointment. Visits will only pertain to the student’s sports physical.
- You will need to bring the sports physical form to your appointment with parent/athlete section completed. The form can be obtained from your school or online at www.ndhsaa.com
- This is a great opportunity to update immunizations! A parent or guardian is encouraged to accompany their child for consent of updating immunizations.
- Baseline Impact Testing will be available at the Beulah Clinic for 9th and 11th grade athletes. There is no charge for Baseline Impact Testing. Walk-ins welcomed until 5 p.m. to be tested.

CCCHC Awarded Grant from the Offutt Family Foundation and R.D. Offutt Company
Coal Country Community Health Center was recently selected as a recipient of a special Community Builder grant from the Offutt Family Foundation and R.D. Offutt Company. The Community Builder program is a unique initiative designed to support charitable organizations and nonprofits that RDO team members are passionate about and serve the greater good in their communities. This year’s program focused on supporting youth mental health nonprofits.
