Temporary Relocation of Patient Entrances. The main entrance for patient visits at Coal Country Community Health Center — Beulah Clinic will be the southside vestibule. Please use this entrance for all services except Physical Therapy. Patient’s needing Physical Therapy, will park and enter the facility from the southeast entrance, enter the parking area from 12th St NW.
