About CCCHC Hazen Clinic
Coal Country Community Health Center ‐ Hazen Clinic began operations in 2016. In April of 2017, the Hazen Clinic relocated into the newly constructed Sakakawea Medical Center. The clinic is equipped with 13 exam rooms, 2 treatment rooms and has access to laboratory services and x‐ray technology through Sakakawea Medical Center. CCCHC Hazen Clinic is staffed with four medical providers, Dr. Rodney Zimmerman, MD, Heather Czywczynski, FNP, Courtney Dean, DNP and Corrie Rogness, FNP.

Dr. Zimmerman has been practicing in the Hazen/Beulah area since November 2009 and currently resides at the Hazen Clinic. Dr. Zimmerman was raised on the family farm, near Streeter/Napoleon, North Dakota. He graduated from the University of North Dakota School of Medicine in Grand Forks, North Dakota. He completed a Family Medicine Residency at the University of Minnesota. He is a diplomate of the American Board of Family Medicine. Dr. Zimmerman served as a Missionary Doctor at Mbingo Baptist Hospital and Banso Baptist Hospital in Africa for almost 30 years.
Dr. Zimmerman and his wife, April, have three children. Dr. Zimmerman enjoys farming, horses, hiking and continues to be interested in mission work.
Heather Czywczynski, Certified Family Nurse Practitioner, graduated from the University of Mary with her Family Nurse Practitioner Degree in April of 2011. She is certified by the American Academy of Nurse Practitioners. Heather graduated with her Nursing Degree from the University of Mary in 2005. She has worked as a nurse and clinical teaching instructor in Nebraska and as a nurse in North Dakota, Texas and California. Heather and her husband Derek live in Beulah. Derek is an optometrist at Beulah Vision.

Courtney Dean, DNP, FNP-BC, began practicing at Coal Country Community Health Center in August of 2017. She currently practices at the Hazen Clinic.
Courtney earned her Doctor of Nursing Practice degree from Montana State University – Bozeman in May of 2017, specializing in Family Practice. She is board certified through The American Nurses Credentialing Center. Prior to this, Courtney earned her BSN from MSU – Bozeman in 2011, and has worked as a medical-surgical and critical care nurse at Billings Clinic Hospital, in Billings, MT. In addition, she has taught classes for and been a full-time faculty member of the College of Nursing at MSU – Bozeman. Courtney also has had the privilege of being heavily involved in overseas medical mission work and refugee care, primarily in Africa and the Middle East.
Corrie Rogness, Certified Family Nurse Practitioner, has been practicing at CCCHC since May of 2007. A native of Watford City, Corrie attended college at UND, NDSU, University of Minnesota, and University of Mary. She received her undergraduate degree at NDSU and her Master of Nursing/FNP at University of Mary.
Corrie transferred from the Beulah Clinic to the Hazen Clinic in December 2016 and is now practicing in Hazen on a regular basis.
