I.M.P.A.C.T. Program
CCCHC and the local School Districts recognized the need for Mental & Physical Health Services for adolescents in Rural North Dakota. Accessibility being priority, a partnership was formed with the schools to create the I.M.P.A.C.T. Program, which stands for Integrating Mental Health Physical Health And Continuity of Care Together.
Our Mission
Enhance and improve the overall wellness of our children through the collaboration of Coal Country Community Health Center, local school districts, and the community.
- Destigmatize mental health and substance use.
- Advocate for students mental & physical health emphasizing that academic performance is directly affected by the student’s health.
- Remove the barriers students face by providing services on-site for mental and physical health.
How it Works
Referrals may be made by school professionals, a primary care physician, the youth bureau, social services, public health, parents, or the students themselves to the I.M.P.A.C.T. Program Coordinator or your local School Counselor.

I.M.P.A.C.T. Program services are provided on-site at the local school districts or any Coal Country Community Health Center. CCCHC’s service area includes Dunn, Mercer and Oliver County and currently we have clinics located in Beulah, Center, Hazen and Killdeer.
All services are private and confidential. We follow strict guidelines related to sharing private health information.

- I.M.P.A.C.T. Program School Nurse
- Evaluates in-school scrapes and cuts, cough, stomach ache, fever, minor school injuries, etc.
- Coordinates virtual visits via TytoCare with CCCHC provider if needed.
- Diabetic Education
- Teaching Diabetes Management to students and school personnel.
- Diabetes Chronic Care Management for students/parents.
- Tobacco Cessation
- CATCH My Breath
- Vaping prevention curriculum offered at the school.
- INDEPTH
- Education based intervention for nicotine dependence.
- Not On Tobacco (N-O-T)
- Tobacco cessation program for teens wanting to quit tobacco.
