Coal Country Community Health Center accepts a variety of insurance plans. Contact your insurance company to verify which CCCHC locations, providers, and services are covered under your specific plan. Make sure you understand your benefits and your health plan’s requirements. Ask about your financial responsibility and how much you will be responsible to pay.
Good Faith Estimate
You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
- You have the right to receive a Good Faith Estimate for the total expected cost of any nonemergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
- Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
- If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
- Make sure to save a copy or picture of your Good Faith Estimate

- Pay Online
- Pay Online with MyChart. CCCHC MyChart users can sign in to see billing and account information, review statements and save payment information.
- Pay by Phone
- Call (701) 873-7788 to speak with a billing representative and pay by phone with credit/debit card. Our billing office hours are 7:30 a.m. - 4:30 p.m. Monday through Thursday and 7:30 a.m. - 4:00 p.m. on Friday.
- Pay by Mail
- Complete the information on the form found on the top of your billing statement. Place with payment in the pre-addressed envelope or mail to:
