Coal Country is proud to provide quality care for our chronic disease patients and their families.

In addition to your medical home visit with your primary care provider, Coal Country has two Registered Nurse Chronic Care Coordinators available to work closely with you and your medical home team. Our Chronic Care Coordinators will provide you and your family with individualized education, tools, and resources to manage your chronic disease with a goal of optimal health and wellness.
